Site risk score

This article explains how to use and edit the site risk score within your Location Risk Assessment (LRA) template. It covers setting the scope, collecting data, viewing risk scores, and managing the risk conclusion to help you accurately evaluate and report on risk at your site.

Note

Required role: Manager or Inspector (depending on action). Creating and editing risk assessments requires Manager role; Inspectors can collect assessment data.

Creating and Setting the Scope of a Risk Assessment

Begin a risk assessment by selecting the parts of your site to be evaluated.

  1. Open the location and go to the Structures tab.
  2. On a structure card, select Create risk assessment. If risk assessments already exist for the structure, use the card's dropdown and choose Create new risk assessment.
  3. In the panel that opens, select the risk assessment activity type configured for your location's control regime.
  4. Set the assessment’s scope by selecting structures, areas, systems, and assets to include. The first item you tick sets the control regime for the entire assessment; other items from different regimes appear greyed out and cannot be added.
  5. Confirm the scope by reviewing the selected items count and clicking Confirm.
  6. Set scheduling details as needed, then create the assessment.

Note: One risk assessment cannot span multiple control regimes or locations. Selections over 500 assets are not allowed.

Collecting Compliance and Assessment Data

Complete data collection using the four data-collection stages within the risk assessment: Location, Systems, Areas, and Assets.

Location-level Assessment

  1. Open the risk assessment and select the Location stage.
  2. Complete the sections shown in the left list, such as Responsibilities, Description, and regime-specific location questions.
  3. Fill in all fields and save each section.
  4. Once all sections are complete, use Mark as done to finish the Location stage.

Note: The types and numbers of location-level sections vary per control regime and are inherited from the location’s regime.

Systems, Areas, and Assets Assessment

  1. From the assessment overview, open the relevant tile: Systems, Areas, or Assets.
  2. For systems, enter required fields such as System Name, Operational Status, and regime-specific parameters. Set the assessment toggle to Satisfactory or Unsatisfactory, then Confirm assessment.
  3. For areas, enter Area Name, optionally override Facility Type and Control Regime, complete parameters, and save changes. Set occupancy if applicable.
  4. For assets, enter identifying details like Asset Number and Network Position, complete parameters, then set the assessment toggle and Confirm assessment.
  5. Override the automatic satisfactory/unsatisfactory evaluation if needed by toggling the status before saving.

Notes:

  • All data collection must include complete parameter entries before assessments can be confirmed.
  • Facility Type and Control Regime are inherited from the parent structure; overriding is possible but does not reassign the underlying regime.
  • Structures themselves do not have a separate compliance form; their compliance is captured through systems, areas, and assets beneath them.

Monitoring Completeness and Progress

Track assessment progress and completeness to know what remains to be done.

  1. Open the risk assessment overview. The header shows a completeness percentage with a circular progress ring aggregating data across all selected items.
  2. The Things to do panel includes the four data-collection tiles: Location, Systems, Areas, and Assets, each showing a status such as Not Started, In Progress, To Review, or Done.
  3. The Things to Review panel includes Logbook & Activities, Control Schema, and Technical Recommendations stages.
  4. Use Mark as done to complete each stage once all data is collected.
  5. The final Risk Conclusion stage unlocks only when all other stages are marked done; completeness reaches 100% when every in-scope object is complete.

Note: For large locations with many objects, completeness percentages may undercount because of system limits on loaded objects.

Reading the Site Risk Score and Risk Conclusion

Review the overall risk level and detailed per-section scores before finalising the assessment.

  1. After all stages are marked done, open the Risk Conclusion stage.
  2. On the right, the semicircular risk gauge shows the overall risk level from Very low to Very high. It reads Unassessed if data is incomplete.
  3. On the left, expand each risk group section to view its numerical score (1–5) and any regime-specific narrative questions.
  4. Use this information to verify the risk score aligns with your expectations.
  5. Fill in the professional conclusion narrative and risk group summaries. Use the Executive Summary and Risk Summary fields to generate draft texts or enter your own. Drafts auto-save.

Note: Narrative and summary fields require assessment-edit access to modify and provide an audit trail for manual overrides.

Editing the Site Risk Score in Your LRA Template

The site risk score is influenced by your control regime’s risk rules and the data collected during the assessment.

  1. Risk scoring rules are configured under your control regime by an admin and translate activity results into quantitative risk scores.
  2. The risk gauge and per-group scores in the Risk Conclusion stage are calculated based on these rules and the assessment data.
  3. Adjusting the score manually is done by entering the scores and narratives in the Risk Conclusion’s per-group sections during the conclusion phase.
  4. Ensure all data collection and review stages are complete to enable editing the risk conclusion.

Important: The risk scoring depends on regime-specific rules and cannot be changed directly by inspectors or managers without proper permissions and admin access to the control regime configuration.

Additional Notes on Using Risk Scores

  • Risk assessments must cover items from a single control regime only; mixing regimes in scope is not allowed.
  • Changing an area or asset’s Facility Type or Control Regime does not alter the site’s overall regime or scoring rules.
  • Scope changes after data collection do not delete existing data but affect completeness counts and risk calculation.
  • Completed technical recommendations and follow-up jobs influence the risk picture and should be reviewed during reassessments.
